He is the youngest of four children born to Isaac and Luna Cohen Barrocas. Albert has an older brother, Salvador, two sisters, Esther and Rebeca. Both of his parents immigrated from Turkey, and met in Cuba, where they married. His father operated a textile factory in Cuba, which was severely impacted Great Depression. His family eventually tried to immigrate to the United States but were unable due to immigration quotas. In 1947, his older brother came to the United States, settling in Atlanta with his mother’s family. A year later, in 1948, Albert’s father passed away while the family was still in Cuba. In August 1954, Albert and his sister, Esther immigrated to Atlanta, Georgia and a year later his mother and sister, Rebeca immigrated.\u003c/p\u003e\r\n\u003cp\u003eAlbert attended Hoke Smith High School and was active in ROTC and various other activities. He graduated from Emory University and Emory University Medical School. While in college he served as president of Hillel and president of Phi Delta Epsilon fraternity. After medical school, he joined the Air Force and completed his medical residency while serving in Texas and Mississippi. Once he was discharged from the military, he worked as surgeon and chief medical officer at Methodist Hospital in New Orleans, Louisiana. After Hurricane Katrina, he returned to Atlanta, Georgia and worked as the chief medical officer at South Fulton Medical Center, later Wellstar Atlanta Medical Center, until he retired in 2018.\u003c/p\u003e\r\n\u003cp\u003eOn June 7, 1964, he married Maxine Handmacher at Ahavath Achim Synagogue. They have four children including Natalie, twins Philip and Isaac and Lane. They also have eight grandsons and one granddaughter. Albert has been very active with the American Society of Parenteral and Enteral Nutrition. He is also an Adjunct Professor of Surgery at Tulane University School of Medicine. He and Maxine belong to Shir Chadash Synagogue in New Orleans and Congregation Or VeShalom in Atlanta. It is a pleasure to speak with you today, Dr. Barrocas, both on a personal and professional basis as a volunteer. I'm going to be asking you just a series of questions to tell us a little bit about yourself, your family, your background. I was born in a small town in Cuba. The province of Havana in the town was called Güines. G-U-I-N-E-S with the two little dots over the U. I forgot what you call that symbol. But how we got there and how I was born there is a rather interesting story just like many of us have that type of heritage. We were able through the efforts of my cousin Dan Maslia to trace our family history on my mother's side, the Cohen side, way back to 1830, 1840. Two brothers, Morris and Eliezer Cohen, were born or lived in Manisa, M-A-N-I-S-A. Turkey. That is just a little bit to the west of Izmir [Turkey] where they eventually ended up and that's where my mother and all our Cohen relatives from that era were born. Then around 1910, Abraham Cohen, my grandfather, made several trips to the United States. A lot of the Sephardic Turkish citizens at that time were afraid, as far as we know, of an upcoming war after the Ottoman Empire was decimated during the two Balkan Wars. There was a fear that the male members would be conscripted into service and so began the need or the search to move elsewhere to migrate to America, which at that time offered great opportunities. There was also internally various upheavals and revolutions, the so-called Young Turks, and also some persecution for both Jews and Muslims at various times. There's are very convoluted history, and I'm not an expert on that, it's just simply what I read. On the other side of the coin, my father, Isaac Barrocas, was born in a town that is a little bit north of Istanbul [Turkey]. Then it was called Constantinople, and it's called Çorlu. I think now they call it Corlu, C-O-R-L-U. I know very little about that migration, their migration. My father was Itzhak, Isaac. They had a sister named Regina that married David Behar. Most of my dealings in terms of research, in terms of family reunions and so on, was driven by the Cohen family that I was much closer to. Around 1910, 1913, I think the entire family moved to Cuba. Let me sort of sidetrack there made up by about the Barrocas. Some of the Barrocas family came to Cuba. Some of Barrocas family came through New York. We have second cousins even in Atlanta who spell Barrocas with one R, mine is two R's. Our cousin Victor Barrocas here in Atlanta has done a lot of research on the Barrocas side. The theory of the discrepancy in the name Barrocas is that if you came through Cuba in Spanish, the double R is part of the alphabet, so Barrocas, as coming through Ellis Island when you ask what is your name, Barocas, one R. I like to throw that in for the historical perspective. Now, if you're, we've seen names of Barrocas with a K from Turkey. That's an aside. Getting back to the Cohens. They come, 1910, my grandfather had a restaurant in Havana [Cuba]. There's a very iconic picture that we have of my uncle David Cohen sitting in the restaurant with my grandmother, Rebecca Cohen, who was the second wife of Abraham. The first wife was Luna. The second wife was Rebecca, who is my grandmother. They're sitting there with Uncle David, who must have been about 10 years old. He's the father of Betty, or Rebecca, Albert, and Solomon Cohen. Solomon is a dentist here. Albert continues to work was very much involved with Happy Herman, the deli and so on and so forth and Betty lives in Augusta [Georgia]. Then they made various trips to Atlanta because by then you had a Sephardic community that was thriving and the synagogue, Or VeShalom was sort of like the magnet, the attraction to many of those individuals. Long story short, they ended up migrating to Atlanta in 1920. Everyone in the family was single except my mother. She was married, and my father had a very successful textile warehouse type of establishment in Calle Muralla, which means wall, in Old Havana. They did not come with them when they arrived here in 1920. In 1921, I think it's also important because this, through Victor Barrocas, I was able to get an archival story of what happened to our uncle Victor Cohen, somewhat of a tragedy. He was there with my grandfather on Broad Street where they were having an annual parade, and they were parked behind a truck. They felt that the future was here, so they sent my brother, who was a Cuban citizen, to live and be supported by the rest of the Cohen family that for the most part has remained in Atlanta all the time. As a matter of fact, Abraham Cohen had thirteen grandkids. Eight from both wives, 18, excuse me, eight males and five females and all of them have been in Atlanta. I was the only one that was a renegade and went off to the Air Force and did other things throughout and then [Hurricane] Katrina brought me back. We can talk about that a few minutes now. My cousin, Betty Cohen Sussman that we talked about earlier, who moved to Augusta, but at least she was in the state. But all the others have been here the whole time. As a matter of fact, the four remaining male cousins get together, we try to get together once a month, but we call the group Ocho Primos, or Eight Cousins. At one time we had seven of us. All of a sudden, we realized that if we called it seven then it was going to be six and so on. The last time I was in Cuba they were refurbishing. I think my father was a one-time treasurer there. I think the Chevet Achim is also where Rabbi Joseph Cohen was a rabbi who became the spiritual leader of our beloved Or VeShalom Synagogue. Christmas time, for instance, we would be invited to people's houses if we wanted to have dinner, but for the most part we celebrated. Thanksgiving was not really Thanksgiving there. Again, being a predominantly Catholic country and very religious, it was different than what I saw when I came here to see Christmas. Because Christmas is, in Cuba, or was, a religious holiday. There were no gifts, there were no trees. They went to mass; they had a big meal with roasted pork and black beans and so on. You got your gifts under the bed January the sixth, which is King's Day. For the other side, for the Christians is the day of the Epiphany, twelve days . . . when the three kings brought gifts. Anyway, so that was different. He made the case that while there were three of us in Cuba, my mother was still in Cuba, we were able to come to the United States. He made the case two of us, myself and my sister Esther would come and the family support [us]. Leave one, Rebeca, with my mother, and we came. With Georgia, I was offered a four-year scholarship, but when I put pencil to paper, the housing and this, that, and the other was not going to be as feasible as going to Emory where I got a partial scholarship. In addition to that, I got what was scholarship slash loan from the state of Georgia that offered subsidies with the promise that you would work in an underserved area once you got out. Once I finished medical school I decided to do a residency. I'm going to get ahead here so that we could finish that loop, and they deferred me. They said fine, do your residency, we'll wait for you. Then I went into the Air Force. Fine, we'll wait until you get through. When I got through with the Air Force, and I did two additional years in the Air Force, at Maxwell Air Force Base, I didn't end up in Atlanta, or the Atlanta area, I ended up in New Orleans [Louisiana]. When I informed them that I was entering practice, private practice, they said, \"Now, we need the money back with interest.\" I paid that back but at least it helped me during that time. I was very happy to pay them back because of the circumstances. You had obligatory service, and the idea was that for healthcare individuals, whether you were already in practice or training, you could request a deferment. They didn't have to serve at all. I wanted to serve in some capacity, but I decided since I wanted to go into surgery, I wanted to have a good foundation. Instead of, in those days we had internships and most people that went into surgery took a surgical internship and then four years of residence for general surgery. I wanted to have a year of medicine, internal medicine, to have a more basic footing before going to surgery. I felt that if I deferred for two years, I would then have that year of medicine and a year surgery, go into the service and practice a little bit in the service so that when I would become a second year resident, if that makes sense, I would be even par with those that had had that extra surgical internship, and that's what I did. I also requested to be in a small hospital and be able to use my Spanish. Now, as you know, if you ask to go to Alaska, they probably put you down in the hottest place in the country. But I got what I requested. On this particular day she was going to have her first exam, and this is before ultrasound, so she had to have a regular x-ray. She goes out to the x- ray, I come home to speak to the housekeeper. Then I went back, and I look at the x -ray and I said, \"Maxine, we're having twins.\" She says, \"No, no, no.\" I said, \"Here's the choice . . . We're either going to have twins or we're going to have one kid with two heads. Which would you like?\" [Everyone laughs] Because that is all we could see [on the] x-ray. Anyway, so that was Isaac and Philip, they're the middle ones. We have Natalie, and then another one came while we are at Keesler [Air Force Base]. I went to Del Rio, Texas, two years, I was what you call a class D surgeon, doing small stuff. If I had a big emergency, then they would air vac those individuals to Wilford Hall in Lackland in San Antonio [Texas], 150 miles, or take them by ambulance. There were only general practitioners in the community that the Air Force didn't recognize as surgeons per se, even though they were doing surgery. Then every six months or so one of the gurus from the big hospital would come and I would arrange, I'd send them the charts and all that of patients. They would assist me doing bigger surgeries like gallbladders or something like that. I had everything arranged through Victor Maslia, he arranged for us to have an apartment here. We're going to come back to Atlanta, and it's a lengthier story, but I found out that I could stay in the Air Force as a second year resident, and so I wanted that because I had seen the results of their residency program, and we went to Keesler in Biloxi [Mississippi]. Following that, I had to pay them back one year, and I decided to stay even another year because I wanted to get through with my certification and all the other things. I was Chief of Surgery at Maxwell Air Force Base in Montgomery. While my wife and I were at the hospital with no lights and no air conditioning, that whole mess. She stayed with Lane, who was living here already. She waited until the school year was finished. She went and rented my house because we were staying here. My hospital was destroyed, not destroyed, but it didn't reopen until about eight years ago. She later became manager of my office or was manager of my surgical office. That went by the wayside, and even before Katrina she had become a director at the hospital dealing with insurance and so on and so forth. As you know, whether you have trauma, broken legs or whatever, you need to have the right precursors for building, for repair, otherwise, you die. Not to go into a big lecture, the fact is that you've heard of the fight, fright, or flight kind of response to a psychological stress or a physical stress. That usually sets a shower of various hormones, steroids and cortisone, things that you've heard about and all of those act to move things out of your muscles, out of your liver to provide nutrient to the repair of tissue and to survive. Unless you provide another source of protein and energy, those individuals will die within about three to four weeks. For years and centuries, people have tried . . . all kinds of things including high dextrose, high glucose enemas with the hope that it would be absorbed or infusing into small veins large amounts of glucose and didn't result in much except make people go into congestive failure, too much fluid. Around mid 1960's, the first breakthrough if you will and it was related to the Mercury Space Center, some people think. They developed these chemically defined solutions that were the components of protein, if you will, but broken down so they were already pre-digested. Once they got it to the stomach and small intestine, they get absorbed. That was the first breakthrough. Having seen, like I said, people die, that we did everything that we could, good surgery, ventilators and all that, but the fact is they could not survive the acute phase of this stress because of the nutrition. At the same time, at the University of Pennsylvania, there was an individual that was a resident that was doing research. His name is Stanley Dudrick. He passed away about three years ago. What he and his group did is that for the first time, two things happened. One, they were able to develop a technique by which they insert a catheter into the major vein without having to do major surgery, the subclavian vein or the other veins because these solutions that they developed, which again were pre-digested if you will, had such a concentration that if they gave it to the peripheral veins, it would just close them off, they would clot off. First they had the technique that they developed based on earlier studies, earlier reports in 1950 by a French surgeon during the war that came across an individual that had all limbs blown off and he was trying to survive in an attempt, he went ahead. Another very interesting story. They've been called hyperalimentation and now it's called parenteral nutrition. Not parental, parenteral meaning intravenous. We have enteral nutrition through tube feedings, and we have the total parenteral nutrition that has been a lifesaver for millions of people through other worlds. You have children that are born where the intestines are twisted and they lose most of the intestine, that have lived 20, 30 years totally on intravenous. Now some of them may be able to sip something just simply for taste, but it just goes right through. In 1970, the report of hyperal, we call it hyperal or TPN [total parenteral nutrition] or PN [parenteral nutrition], was just being reported. Dudrick and his group reported in 1969. I cannot tell you how many patients, personally, I've seen plus the many others that I've heard about through our society, which is the American Society for Parenteral and Enteral Nutrition. I've been on the board there twice, and many, many things. Positions. Presentations. Publications. I remain still active in that area, very interested. As a matter of fact when I was in Potomac just last week I went to their office that is in Silver Spring, Maryland and we're getting ready to celebrate our fiftieth anniversary, but the idea is that now you have provided for the last 50 years hope to people that would die otherwise. Most of the time this ability to sustain the individual is temporary, just during the acute phase. One observation that a critical care doctor that is a member of our society, he's in Canada, big on critical care, reminds us is that, even if you survive a critical illness and you get discharged from the hospital, the majority of the time, you don't have a survivor. He calls them victims because at the end of a year, they still [are] not what they used to be. In part is because they didn't receive their proper nutrition on the front end. The best way I think that I can explain this to you without going into great detail is that if you don't provide glucose, which is for your energy, all the glucose in your body disappears in 24 hours in the form of glycogen. The body cannot, it can use some of your fat, but your brain cannot. It's got to come up with another solution for new molecules of glucose. It comes from your muscle. Not just the biceps, the diaphragm, the heart, it's all muscle. People have referred to this as proteinaceous piracy or auto cannibalism. You're chewing up on your own meat. If you think in those terms to make it very basic this is what you're trying to prevent. On admission to hospitals, surveys have indicated that between 30 and 50 percent, particularly of elderly adults, are malnourished. At one time, those kids, two of them had pneumonia, they had to be in the hospital, the other one had to be at home, etc . . . Those values that then get molded by life experiences. We didn't have time to go into it because one of the other things that, as part of the nutrition aspect, is who should be fed and who should not. It becomes the ethical perspective. I've become very interested in that. Been interested since 1968 when the twins were born. One of them on New Year's Eve at six weeks old had severe meningitis, was not expected to live, had a cardiopulmonary arrest, tiny little thing, in that 25-bed hospital in Del Rio. You've got to take him to the hospital.\" He did a spinal tap. I was there, where you put a needle in the back. Out came pea soup under pressure. That's how much pus there was. He says, \"Doesn't look good, but we're going to put him on antibiotics, go home.\" Got a call at five in the morning, \"Come right away.\" As I was walking in, he's there, pumping on the chest and blowing with a mask and all that. You can imagine how that was. He stayed on the ventilator for three days. We called the big hospital, or he called the big hospital because they had pediatric infectious disease. The word came; there's no need to transfer him. He can die there just as well as he can die here. But then as the years went on and I got involved with nutrition and nutrition at the end of life, because there comes a time, as much as I promote nutrition, there comes time when you have to respect the dignity of the individual. You have to be honest with them and give them the information they need to make appropriate decisions and for instance in patients for whom there is no hope or for whom have severe dementia you do more damage than good. You weigh the benefits against the risks and if you provide good information to the family and following the full current principles of ethics in Western society. Autonomy, beneficence, non-maleficence, and derivative justice. They're all weigh the same, but autonomy means that anyone that has the ability to be competent is entitled to their decisions. But it has to be with good information. You will be surprised how many people don't want to be artificially fed when they're already gone. In a lot of times, particularly when you have tube feedings, they try to reach for the tube, so they hold them down. They restrain them. Is that the way you want to go in your last days? Plus, the fluid accumulates both in the lungs. Then you have to go to the bathroom a lot, and you can't get out of bed. Then you have incontinence, that leads to pressure sores. That again, a life-changing experience. The other life-changing experience of course was Katrina because for a while that we didn't think we're going to make it. My wife stopped eating in part because or drinking in part because she didn't want to have to go to the bathroom because . . . it was all dark, there was no way to flush. All this went for about five days.
